  • South Tyneside & Sunderland NHS Foundation Trust SPA - Congratulations on your pregnancy! If you live in the South Tyneside and Sunderland areas please use this self-referral form to book for our maternity services. If you live out of area, you will also need to contact your local midwife at your GP surgery. It is important that you contact us as soon as you know you are pregnant to ensure you get the best possible health support and are able to access free prescriptions, dentistry and other support so your care is personalised to your individual needs.
  • York and Scarborough SPA - Congratulations on your pregnancy. If you are wanting to have your pregnancy care at Bridlington, Malton, Scarborough, Selby, Whitby or York please use our online referral. To help us process your referral quickly please use your NHS number, this can be found on the NHS app or from your GP. Your initial contact will be a phone call (which might be from an unknown number).  Your booking appointment should happen before you're 12 weeks pregnant.
  • Oxford University Hospitals Foundation Trust pregnancy self-referral - Congratulations on your pregnancy and thank you for choosing Oxford University Hospitals - we look forward to supporting you! Please complete this self-referral form after 6 weeks of pregnancy to notify your local Community Midwives who will arrange your first appointment with the team.   • West Herts SPA - Welcome to our maternity unit at West Herts Teaching Hospitals NHS Trust. We provide personalised and high-quality maternity care for women living in Hertfordshire and North London. Please include your NHS number on your referral. We will inform your GP that you are pregnant and to share relevant health information. We will also liaise with the local authority if your midwife thinks you will benefit from extra support. Please let your midwife know at your booking appointment if you do not have permanent access to a digital device or network data.
  • Dorset Referrals - You can refer yourself to Dorset maternity services by completing this self-referral form. If you don’t wish to have your baby at a Dorset hospital, please directly contact the hospital you wish to birth at. You may still receive some of your care from Dorset midwives. If you are considering a homebirth, please complete the self-referral form. You will then receive a booking appointment, which will happen when you’re around 8 - 12 weeks pregnant, or as soon as possible if you believe that your pregnancy is further along. There is no need to see your GP unless you have existing health problems. If you do have any existing health problems, it’s a good idea to see your GP when you find out that you’re pregnant, so that they can review any medication you’re taking and see if you need any additional care.
  • Northumbria Healthcare SPA - Women in the North Tyneside and Northumberland areas will be able to self refer their pregnancy to Northumbria Maternity Services using the portal regardless of which hospital they wish to have their baby at.

  • Bristol, North Somerset and South Gloucestershire SPA - Welcome to Bristol, North Somerset and South Gloucestershire (BNSSG) maternity services. Please use this link if you are pregnant and wish to refer for antenatal maternity care at either North Bristol NHS Trust NBT (Southmead or Cossham) or University Hospitals Bristol and Weston NHS Foundation Trust UHBW (St Michael's or Weston General Hospital). The following form requires you to provide personal information including your GP details and your NHS number.   • Somerset SPA - If you are more than 6 weeks pregnant you can use this maternity referral form to tell us about your pregnancy.  Your pregnancy care will be provided by a team of midwives based on which GP surgery you are registered with. You can choose to birth your baby at either Yeovil or Taunton, your midwife will discuss this with you during your appointments. Once you have submitted your self-referral you should receive a confirmation e-mail to let you know it has been received. BOOKING FOR OTHER HOSPITALS: If you wish to have your baby at another hospital please do not complete this form and please contact the hospitals directly on the numbers provided below... Dorchester: 01305 253315 Bath: Paulton Birth Centre on 01761 412107 or Shepton Mallet Birth Centre on 01749 342378. St Michaels Hospital in Bristol : Please be advised that are no longer accepting referrals from Somerset residents at this time.   • Harrogate Referral Location - If you are pregnant and are considering giving birth at Harrogate, please get in touch as soon as possible so that we can start planning your care together. You may also complete this from at any stage in pregnancy if you wish to transfer your care to Harrogate from another maternity care provider. You can also complete this form to refer a pregnancy on behalf of a woman. Our aim is make sure you have access to the right care from day one. Harrogate will triage your referral based on the information you provide and the hospital will contact you in the next few weeks to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away. Some people need to take an increased dose of folic acid in the first 12 weeks for pregnancy to reduce the risk of neural tube defects such as spina bifida, please read the information on the link below.   • Hull Maternity Services SPA - Congratulations on your Pregnancy. Hull Women & Children’s Hospital provide high quality, inclusive care for pregnant women and their families in Hull and the East Riding of Yorkshire. If you would like to book for your antenatal care with us please complete our online referral below. If you haven't started already we suggest that you start taking Folic Acid, if your BMI is under 30 you need to take the standard 400mcg dose which is available over the counter or in pregnancy vitamins, if your BMI is 30 or above you will need to contact your GP to obtain a 5mg dose of Folic Acid, you will need to take this until you are 12 weeks pregnant. We also suggest everyone takes a 10mcg dose of Vitamin D daily for the duration of your pregnancy, this is available over the counter or in pregnancy vitamins. Should you have any problems completing this referral on line please contact our Community Admin Team on 01482 382658 who will be happy to help you. Hull University Teaching Hospitals offers you the choice of where you would like to have your baby, these include The Alongside Midwifery Led Unit (AMLU - Fatima Allen Birthing Centre), the labour ward within the hospital or your home (Homebirth). All of these options will be discussed in detail at your booking appointment. In the meantime if you experience any pain or bleeding and are worried about your pregnancy before 16 weeks, please contact your GP/Walk in Centre or 111 straight away. You do not need to inform your GP that you are pregnant however if you do have any existing health problems, it’s a good idea to make an appointment to see your GP, so that they can review any medication you’re taking and see if you need any additional care. You can also use this to refer a pregnancy on behalf of someone else.
  • SHIP SPA - When completing the self referral questions if you are unsure of your exact LMP (Last Period), then please just add an approximate date. Women wanting their maternity care at either: Portsmouth Hospitals University NHS Trust, Andover War Memorial Hospital, Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital, University Hospital Southampton or Isle of Wight NHS Trust, are now able to refer their pregnancy using this portal. We are working together as SHIP LMS (Southampton, Hampshire Hospitals, Isle of Wight, and Portsmouth Trusts Local Maternity Services) to provide safe maternity care for you.
  • North Tees and Hartlepool SPA - Congratulations! Please tell us about your pregnancy by completing the online referral form and a member of our team will be contact. You have a choice as to where you receive your care and where you wish to have your baby. If you are assessed as low risk throughout your pregnancy, you have the opportunity to be cared for at the low risk Midwifery led unit in Hartlepool called the Rowan Suite. If you would prefer this option the chose the Rowan Team when asked for the preferred location of care on the online form. If you believe you are >20 weeks pregnant, please contact the community team urgently on the following contact numbers to arrange a booking appointment.   • New Cross Maternity - Congratulations on your pregnancy! Whether this is your first or you've already experienced Maternity services, we look forward to supporting you to access the care you want throughout your pregnancy journey. If you want any part of your care with us at New Cross, please complete the referral form by clicking on the green 'begin self-referral' button below. Complete the form as fully as possible - some fields are mandatory and you wont be able to continue without completing them. Once we receive your completed referral, we will triage it based on the information you have provided and a Midwife will be in touch to arrange your booking appointment. In the meantime, if you have any concerns about your pregnancy please speak to your GP. If you are Diabetic, it is important that you start taking Folic Acid 5mg as soon as possible.   • Tayside SPA - **THIS SERVICE IS FOR WOMEN LIVING IN DUNDEE, PERTH & KINROSS AND ANGUS** Please use this form to begin your maternity care.   • Royal United Bath Self Referral (SPA) - RUH Maternity is the umbrella for midwifery and consultant services at Bath, Chippenham, Frome, Paulton and Trowbridge. Your pregnancy care will be coordinated by midwives based at our community units. If you live outside our catchment area, e.g. in BRISTOL or KEYNSHAM, but would like to birth with us, PLEASE DO NOT REGISTER here. You will need to register with your local community midwives and they will refer you later in pregnancy. Thank you.   • Ashford and St Peter's SPA - If you are more than 6 weeks pregnant you can book your maternity care directly with Ashford and St Peter’s maternity services in Surrey by completing the self-referral form. If you have any concerns about your pregnancy please call your GP (NHS Direct - Call 111 out of hours) or attend Emergency Department if severe pain or very heavy vaginal bleeding.
