- Northern Lincolnshire and Goole NHS Trust Pregnancy Self Referral - Northern Lincolnshire and Goole NHS Foundation Trust NLAG is an NHS service providing a single point of access for all maternity referrals in Northern Lincolnshire and Goole area. NLAG will triage your referral based on the information you provide and the hospital will contact you to start your antenatal care in due course, with your first appointment with the midwife being arranged for approximately 8 weeks of pregnancy . If you haven't started already we suggest that you start taking Folic Acid, if your BMI is under 30 you need to take the standard 400mcg dose which is available over the counter or in pregnancy vitamins, if your BMI is 30 or above you will need to contact your GP to obtain a 5mg dose of Folic Acid, you will need to take this until you are 12 weeks pregnant. We also suggest everyone takes a 10mcg dose of Vitamin D daily for the duration of your pregnancy, this is available over the counter or in pregnancy vitamins. You do not need to inform your GP that you are pregnant however if you do have any existing health problems, it’s a good idea to make an appointment to see your GP, so that they can review any medication you’re taking and see if you need any additional care.

When you find out you’re pregnant, you will need to book your first appointment with one of our midwives. This is called your booking appointment. You need to register your first appointment with your midwife using our online booking form as early as possible in your pregnancy. If you are more than 8 weeks pregnant or unsure of your dates, you need to register as soon as possible. Once you have completed the online booking form, a midwife will contact you by 9 weeks of pregnancy to book your first appointment. This call may appear as an unknown number.

- West Herts SPA - Welcome to our maternity unit at West Herts Teaching Hospitals NHS Trust. We provide personalised and high-quality maternity care for women living in Hertfordshire and North London. Please include your NHS number on your referral. We will inform your GP that you are pregnant and to share relevant health information. We will also liaise with the local authority if your midwife thinks you will benefit from extra support. Please let your midwife know at your booking appointment if you do not have permanent access to a digital device or network data.
- Dorset Referrals - You can refer yourself to Dorset maternity services by completing this self-referral form. If you don’t wish to have your baby at a Dorset hospital, please directly contact the hospital you wish to birth at. You may still receive some of your care from Dorset midwives. If you are considering a homebirth, please complete the self-referral form. You will then receive a booking appointment, which will happen when you’re around 8 - 12 weeks pregnant, or as soon as possible if you believe that your pregnancy is further along. There is no need to see your GP unless you have existing health problems. If you do have any existing health problems, it’s a good idea to see your GP when you find out that you’re pregnant, so that they can review any medication you’re taking and see if you need any additional care. - Somerset SPA - If you are more than 6 weeks pregnant you can use this maternity referral form to tell us about your pregnancy. Your pregnancy care will be provided by a team of midwives based on which GP surgery you are registered with. You can choose to birth your baby in hospital at either Yeovil or Musgrove Park Hospital, or at home, your midwife will discuss this with you during your appointments. Once you have submitted your self-referral you should receive a confirmation e-mail to let you know it has been received. If you wish to have your baby at another hospital please discuss this with your midwife at your booking appointment who can refer you to your chosen hospital where appropriate. Please be advised St Michaels in Bristol are not accepting out of area women unless it is clinically indicated due to capacity issues.
- Harrogate Referral Location - If you are pregnant and are considering giving birth at Harrogate, please get in touch as soon as possible so that we can start planning your care together. You may also complete this from at any stage in pregnancy if you wish to transfer your care to Harrogate from another maternity care provider. You can also complete this form to refer a pregnancy on behalf of a woman. Our aim is make sure you have access to the right care from day one. Harrogate will triage your referral based on the information you provide and the hospital will contact you in the next few weeks to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away. Some people need to take an increased dose of folic acid in the first 12 weeks for pregnancy to reduce the risk of neural tube defects such as spina bifida, please read the information on the link below.
