Self-refer your pregnancy to Walsall Manor Maternity

If you have changed your name or address and not informed your GP then please do not make a referral using this method until the GP has updated their system, please note you will also need your NHS Number.

Walsall Manor Maternity is an NHS service providing a single point of access for all maternity referrals in the Aldridge, Bentley, Bescot, Birchills, Blakenhall, Bloxwich, Brownhills, Darlaston, Great Barr, Leamore, Pelfry, Pelsall, Pleck, Rushall, Shelfield, Streetly, Walsall Town Centre, Walsall Wood and Willenhalll.. The aim is to make sure you have access to the right care from day one. Walsall Manor Maternity will triage your referral based on the information you provide and the hospital will contact you to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away.

Please complete the form and a member of the community team will be in touch with you to arrange your first appointment with us. The following form requires you to provide personal information including your GP details and your NHS number. If you don't know your NHS number please visit https://www.nhs.uk/nhs-services/online-services/find-nhs-number/ Please allow a period of up to 7 days for the receipt of a callback to arrange your initial appointment. We are currently managing a considerable volume of referrals on a daily basis.
