Self-refer your pregnancy to Gloucestershire Maternity

If you have changed your name or address and not informed your GP then please do not make a referral using this method until the GP has updated their system, please note you will also need your NHS Number.

Gloucestershire Maternity is an NHS service providing a single point of access for all maternity referrals in the Gloucester Stroud Cheltenham Forest of Dean Cirencester Tewkesbury North Cotswolds. The aim is to make sure you have access to the right care from day one. Gloucestershire Maternity will triage your referral based on the information you provide and the hospital will contact you to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away.

Congratulations on your pregnancy! Welcome to Gloucestershire Maternity Services. Please complete the online form to refer yourself into the Maternity Services. When you find out you’re pregnant, you will need to book your first appointment with one of our midwives. This is called your booking appointment. You need to register your first appointment with your midwife using our online booking form as early as possible in your pregnancy. If you are more than 8 weeks pregnant or unsure of your dates, you need to register as soon as possible. Once you have completed the online booking form, a midwife will contact you by 9 weeks of pregnancy to book your first appointment. This call may appear as an unknown number.
