Self-refer your pregnancy to Gateshead Health Maternity

If you have changed your name or address and not informed your GP then please do not make a referral using this method until the GP has updated their system, please note you will also need your NHS Number.

Gateshead Health Maternity is an NHS service providing a single point of access for all maternity referrals in the Gateshead, Tyne and Wear.. The aim is to make sure you have access to the right care from day one. Gateshead Health Maternity will triage your referral based on the information you provide and the hospital will contact you to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away.

Congratulations on your pregnancy! If you live in Gateshead and are registered with a Gateshead GP please use this self-referral form to book for our maternity services. If you live outside of Gateshead, you will need to complete the referral for the hospital in the area you live, no matter where you would like to give birth. To complete this referral, you will need to know your NHS number and the name of your registered GP practice. You can find this information on your NHS app account or by contacting your GP practice directly. If you cannot find your GP on the list, please check other maternity services in the area, as it is likely you need to refer to another service. If you are not registered at a GP practice currently, please select “unknown” on the list. If you are struggling to complete this form or are unable to refer yourself, please email ghnt.community.discharges@nhs.net or call your GP practice. Once a referral has been submitted, you will receive a confirmation email. If you have not received this email within 24 hours, please repeat the referral. Please note that a midwife will contact you shortly to arrange an appointment between 8-10 weeks pregnant. If you have not heard anything within that time frame, please call the community midwife base on 0191 445 5306. If you have any concerns or questions in the meantime, please contact your GP.
