Click here to inform the NHS of your pregnancy. It's quick and easy and just takes a couple of minutes.

Congratulations on your pregnancy. You can choose who provides your care and where you give birth. United Lincolnshire Teaching Hospitals NHS Trust (ULTH) are proud to offer maternity services at Lincoln County Hospital and Pilgrim Hospital, Boston. If your GP is in Lincolnshire, regardless of where you would like to have your baby then please use this referral form. If you live in Louth, Mablethorpe, Alford, Stamford or Bourne, you can find information and self-referral options for different hospitals on the Better Births website (www.betterbirthlincolnshire.co.uk/where-to-give-birth). If you're still unsure on how to register your pregnancy in your area, please speak to your community midwife via your GP surgery. If your GP is not in Lincolnshire, please contact your community midwife via your GP surgery. There are three birth options at ULTH; you can give birth at Lincoln County Hospital, Pilgrim Hospital, Boston, or at home, if your pregnancy is low risk. If you have a higher chance of complications, you will be supported by your midwife and consultant to plan the place of birth. Our two hospitals have midwives, obstetricians, paediatricians, and anaesthetists to help care for you and your baby. You can change your mind at any time about where you have your care or place of birth.

You can also use this to refer a pregnancy on behalf of a woman.
